Vendor Checklist for Events at the Museum

In order to make for a well-planned event, please fill out this form and return it to Tony Pizzo or Wendy Melton at the Museum with the balance of your rental fee and Certificate of Insurance two weeks prior to your event. (Returning this checklist earlier is welcomed.) If possible, please indicate the day and time when vendors are to be expected at the Museum. Please call the Museum with any questions.

If at all possible, we would prefer your wedding planner or contact person to arrange a meeting at the Museum of all or most of the vendors one to two weeks before the event to discuss coordination of the event. This is especially helpful for weddings / receptions.
